Ive seen similar on Laptops with a defect in the power rail. If you know how to, check the MOSFET are and the capacitors, its likely ones gone open or shortcircuit.
Ive had this when someone has used an OEM charge for it, rather than a makers own brand. Was a while back though. I havent seen this type of erro for a long time.
